How the Corrected Calcium Calculator Works

About 40–45% of the calcium in your blood is bound to the protein albumin and is not biologically active. When albumin is low, the total calcium measured by the lab reads low even if the active (ionized) calcium is normal. The corrected calcium calculation estimates what the total calcium would be if albumin were normal. Enter your measured total calcium (mg/dL) and serum albumin (g/dL), and the tool returns the corrected value.

The Formula

This calculator uses the widely taught Payne formula in conventional US units:

Corrected calcium = measured total calcium + 0.8 × (4.0 − albumin)

where calcium is in mg/dL and albumin is in g/dL. The constant 4.0 g/dL is the assumed normal albumin, and 0.8 is the adjustment per 1 g/dL drop in albumin.

Worked Example

A patient has a total calcium of 8.2 mg/dL and a low albumin of 2.5 g/dL:

  • Corrected Ca = 8.2 + 0.8 × (4.0 − 2.5)
  • = 8.2 + 0.8 × 1.5
  • = 8.2 + 1.2 = 9.4 mg/dL

So although the measured calcium of 8.2 looks low, the corrected value of 9.4 mg/dL sits within a typical reference range — the low reading was largely explained by the low albumin.

Limitations and Notes

The albumin-correction formula is a rule of thumb, not a precise measurement. It performs poorly in settings such as critical illness, chronic kidney disease, acid–base disturbances and abnormal protein states. When an accurate value is essential, a directly measured ionized calcium is the gold standard and does not require correction. Corrected calcium is an estimate of what a person's total calcium would be if their albumin were normal. About half of blood calcium is bound to albumin, so when albumin is low, the measured total calcium reads low even if the active (ionized) calcium is fine. The correction adjusts for this.
